1. Nature of the concern for which this plan is being written: Stephanie has Multiple Chemical Sensitivity (MCS) as well as asthma/allergies. The asthmatic reactions can be exercise-induced, chemically induced, or related to her allergic triggers.
2. Describe how the concern affects a major life activity: (caring for one's self, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning, & working.)
b. During exercise, Stephanie may have an asthmatic episode. (see attachment from her doctor)
c. Absences that result from her condition are a concern in that they affect her learning, and possibly her grades.
c. Stephanie's team of teachers will meet with Mrs. Hunt at the beginning of each term to look at the school calendar and determine if notification should be sent out prior to school activities.
d.Stephanie will be allowed to self-limit activities that involve running or other strenuous exercise.
e. Should Stephanie miss more than two days in a row because of illness related to her asthma, a tutor will be provided to assist with make-up work. A reasonable amount of time will also be provided for Stephanie to complete assignments and projects.
